In preparation for your upcoming visit, we kindly request that you complete the following items in advance to help ensure a smooth and efficient appointment:
✅ Required Forms:
- New Patients: Please complete the New Patient Information Form and read the Post-Visit Summary Form. Additionally, we ask that you view the Disclaimer Video, which is located at the bottom of this page.
- All Patients (New and Returning): Please complete the Test Requisition Form to inform us of any tests you wish to have performed during your visit.

All Patients (New and Returning):
To facilitate your care, please fill out the Test Requisition Form, indicating any tests you wish to have performed during your scan.
Important: In addition to completing the necessary paperwork, a swab sample will be required from all clients, both new and returning to detox.
